GLP-1 alone or GLP-1 with a workup: why the labs change the outcome

GLP-1 medications work. They also plateau, regain weight when stopped, and sometimes do not work for patients who are clearly insulin-resistant. The difference between a prescription and a protocol is what gets investigated.

What GLP-1 does, simply

Semaglutide, tirzepatide, and the rest of the GLP-1 class mimic an incretin hormone that slows gastric emptying, increases satiety, and improves insulin signaling. Most patients lose 15 to 22% of body weight in the first 9 to 12 months. The data is real.

Where prescription-only protocols fall short

The patients who plateau, regain quickly, or never respond well usually share one of three labs we routinely find:

  • Untreated insulin resistance with a healthy fasting glucose. Fasting insulin > 8 with normal glucose is the early metabolic syndrome picture. GLP-1 helps but the underlying insulin signaling problem needs separate work.
  • Functional thyroid dysfunction. A TSH above 2.5 with low Free T3 makes weight loss two to three times harder. Optimizing thyroid often unlocks the plateau.
  • Elevated inflammatory markers. hs-CRP above 1.0, often paired with high ferritin, points to chronic low-grade inflammation that drives leptin resistance. Treat the inflammation, the weight follows.

What we run before the prescription

Before we write a GLP-1 prescription, we run:

  • HbA1c, fasting glucose, fasting insulin, HOMA-IR (calculated)
  • Full thyroid panel (TSH, Free T4, Free T3, Reverse T3, TPO)
  • Sex hormones (testosterone or estradiol depending on sex)
  • hs-CRP, ferritin, homocysteine
  • Lipid panel including ApoB
  • Vitamin D, RBC magnesium, B12

Why protocols win
